If your organization qualifies for tax-exempt purchases, please complete the process before placing your order. Once an order has been placed or invoiced, we are unable to refund or remove taxes.

To request tax-exempt status:

Please fill out the form below and upload your valid tax exemption certificate.

Before submitting:

• Include your MedikoHub account number (if you already have one).
• Ensure the company name and address on the certificate match the details in our account.
• Submit only legible, current certificates with accurate state and business information.

Once approved, your account will be marked as tax-exempt and future orders will not include sales tax.

Important:

• Taxes cannot be refunded for orders placed before tax-exempt status is approved.
• Processing takes approximately 1–2 business days after submission.
• For multi-state customers, please upload certificates for each applicable state.
